Primary Nurse Aide Training Instructor Login Request Form Application

Please complete this form if you are an Instructor in a California Department of Health (CDPH)-approved Nurse Aide Training Program in California and would like to access TMU©.  TMU© is the testing software platform that the nurse aide competency evaluation will be conducted through with D&SDT-HEADMASTER. 

You will then be able to enter new students' training records into the TMU© software and complete their training when they have successfully completed your program.  Once a candidate's training record is completed, you will be able to register them for their state competency evaluation testing.

NOTE:  If you are an instructor for multiple training programs, please list each program name and program approval number under the Nurse Aide Training Program Affiliation(s) section.  We will verify that you are a primary instructor with the training program coordinator(s).  Please provide the training program coordinator name and email address. If you are an instructor for multiple programs, please provide the training program coordinator name(s) and email(s) with the corresponding Programs.
